Wildcat Community Fitness Center Policy

REQUIREMENTS- MUST LIVE IN THE SLATER SCHOOL DISTRICT, ARE CURRENTLY PAYING TUITION TO THE SLATER SCHOOL DISTRICT, OR PAY PROPERTY TAX IN THE SLATER SCHOOL DISTRICT (MUST SHOW PROPERTY TAX RECEIPT)

 Hours of Operation/Fee-4:00 a.m.-7:30 a.m./5:00 p.m.-11:00 p.m.-Priority will be given to SHS students during school hours and practice times.  $25-person/$50-family (up to 5 immediate family members) Weekend and Holiday hours of operation for the community will be 4:00 a.m. – 11:00 p.m.

 Code of Conduct-I agree to abide by all policies and procedures of the Slater School District and understand that failure to act in accordance with these rules may result in suspension/cancellation of access to the Wildcat Community Fitness Center.  I further acknowledge that I have reviewed all policies and procedures that are located at www.slaterschools.net.

 Property Loss-I understand that the Slater School District is not responsible for personal property lost, damaged or stolen while using the Wildcat Community Fitness Center. 

 Criminal History-I acknowledge that it is the policy of the Slater School District to deny membership to any individual accused or convicted of any crime involving sexual/physical abuse or, is or has been a registered sex offender.  The Slater School District will utilize the Raptor technology to search against the sexual offender registry list prior to membership.  The District will periodically check its membership records for criminal history.  It is my responsibility to communicate within 48 hours to the Slater School District should a violation occur to ensure the safety of everyone using the facility.  Failure to communicate a violation within the designated timeframe could result in permanent suspension. 

 Proper Attire and Hygiene-Wearing exercise clothing such as shorts, sweats, tank-tops, and athletic shoes is required in working out in the Wildcat Community Fitness Center.  DO NOT wear blue jeans, or other clothing that have external metal parts or rigid seaming.  These will cause damage to the upholstery on the equipment.  Shirts and clean shoes must be worn at all times!

 Tobacco & Alcohol Free Area-Slater School District helps promote healthy lifestyles; use of tobacco products (cigarettes, e-cigarettes, cigars, pipe, chewing tobacco, or snuff) and alcohol are not permitted on school grounds.

 Age Requirements-For safety reasons during non-school hours, no one under the age of 11 will be allowed in the Fitness Center.  Children 12 and over will be allowed if accompanied by a parent until they graduate.  No student is allowed unaccompanied by an adult. Students should not have possession of key fobs. The machines are not toys or jungle gyms and should not be used by an elementary age child.

 Member Assumption of Risk and Release-I understand and accept that the risk of injury from using facility equipment is significant, including the potential for permanent injury and or death, and I knowingly and freely assume all such risks, both known and unknown.  I acknowledge that this is an unsupervised facility and I assume all risks associated with using exercise equipment and exercising alone.  I hereby release, indemnify, and hold harmless the Slater School District.

 Safety Notices

·         24-hour recorded video surveillance and member access usage is logged.

·         You must not bring in guests at any time.  This would result in a policy violation which will leave you subject to suspension or cancellation of membership.

·         No one else is authorized to use your access card.  You must notify the Slater School District immediately of a lost or stolen card (660-529-2278). 

·         Horseplay, vulgar language, abuse of the equipment, working out while intoxicated, or other inappropriate behavior will not be tolerated and may result in suspension and or cancellation. 

·         Membership grants access to the Wildcat Community Fitness Center ONLY. 

 Maintenance of Facilities-The facility may be temporarily closed for a period of up to 2 weeks each year for maintenance purposes. 

 Access Card-Initial access card (1 per family) will be free of charge.  Any additional or replacement cards will cost $10.00 each.  All access cards will be deactivated annually; reactivation will take place upon signing a new agreement for effective July 1st. 

 Membership Use- Membership is for Individual use ONLY.  The Wildcat Community Fitness Center is NOT to be used for business use or implied business.  Membership will be revoked if intentions are questionable.
